The Bridge Renewal Trust, in partnership with Haringey Clinical Commissioning Group and Haringey GP Federation, are delivering the care navigation workstream of the Haringey West Care Closer to Home Integrated Network (CHIN). The Haringey West CHIN project will provide a multi-disciplinary team integrated with (currently) four general practices and home/scheme staff to assess and meet the needs of older people in the 8 residential care homes and extra care housing schemes in West Haringey which are served by the CHIN’s GP practices.

Are you a passionate and dynamic individual with excellent organisational and interpersonal skills; with a proven track record of working in a multi-disciplinary team, working closely with general public, GP practices and on-site staff – preferably in care homes or their own homes?
We are currently recruiting a Care Navigation Co-ordinator who will be responsible for helping residents to make full use of the community and voluntary sector resources available locally. The key requirements for this post are experience of working in a multi-disciplinary team and being familiar with voluntary and community services to respond holistically to patients’ needs.
We are looking for someone who is: self-organised and effective in co-ordinating others and possesses multi-disciplinary team working skills. Must be able to: receive referrals and following on from the assessment and care plan, or from interactions with other professionals such as the GP, focus on helping residents to make full use of local community and voluntary sector resources available locally, collate information to ensure that the service has access to the most up to date information and advice including a summary of voluntary and community resources, establish and maintain relationships with the voluntary and community, statutory and other agencies and work with the CHIN members to develop new opportunities, and problem solve to improve access.
