Posted by: Jenny Carter on April,19th,2017

About our service:

We are continuing to deliver the Home from Hospital Service which provides much needed practical and emotional support to patients aged over 18 years old to return home safely from hospital on discharge. We accompany the patient home and provide up to three home visits for up to four weeks after discharge to prevent unnecessary re-admission. The Service is funded by Haringey Council and Haringey CCG for the next 3 years.
